To prescribe certain forms for the purposes of the Trade Unions Act, Cap. 161.

1. Register of Trade Unions

The Register of Trade Unions to be kept by the Registrar shall be in the form set out in Schedule 1.

2. Application for registration

An application for registration of a trade union shall be lodged by not less than seven members of the proposed trade union in the form set out in Schedule 2.

3. Certificate of registration

A certificate of registration of a trade union shall be in the form set out in Schedule 3.

4. Application for change of name of trade union

An application for the registration of the change of name of a trade union shall be in the form set out in Schedule 4.

5. Certificate of registration (change of name)
A certificate of registration (change of name) shall be in the form set out in Schedule 5.

6. Application for the registration of an amalgamation of trade unions
Application for the registration of an amalgamation of 2 or more trade unions shall be in the form set out in Schedule 6.

APPLICATION FOR REGISTRATION OF AN AMALGAMATION
OF TRADE UNIONS

We the undersigned hereby apply to the Registrar of Trade Unions for consent to amalgamate our trade unions in accordance with the provisions of section 43 of the Trade Unions Act, Cap. 161 and state as follows:
